The Problem
Handwritten prescriptions forwarded on Telegram. No version history. No way to know if a dosage changed.
Each hospital has its own portal. Trend tracking across visits requires manual comparison across PDFs.
The cardiologist does not know what the endocrinologist prescribed. Drug interactions get missed — until they cause harm.
Infrastructure
UHI is India's open protocol for health services -- the same idea as UPI, applied to healthcare. Aayu is built directly on this stack, which means your parent's records from any ABDM-linked hospital or lab can flow into one place with their consent.
Ayushman Bharat Health Account -- a 14-digit identifier that links a person's health records across all hospitals, labs, and pharmacies. 847 million IDs created. Your parent's Apollo visit, the local clinic prescription, and the diagnostic report are all accessible through one number.
UHI is the open network layer that lets any compliant app discover doctors, book consultations, and access health records -- without being locked into one hospital's portal. Governed by the National Health Authority. Think of it as the routing layer beneath every health interaction.
Before any record moves, the patient approves the request -- what data, from which provider, to whom. The Health Information Exchange and Consent Manager is the layer that enforces this. Aayu uses it to ensure family members see only what the patient has explicitly permitted.
Position
ABDM-linked apps in India are strong on infrastructure. Most stop at storing records. Aayu is built for the person who is not the patient -- the adult child managing a parent's care from another city.
Every data access in Aayu is logged to a consent table, tied to the patient's ABHA ID. The HIE-CM layer ensures no family member or doctor can pull records without an explicit approval event on record. Today this lives in Supabase with full timestamps and role attribution.
Records are versioned and never deleted. When a prescription changes, the old version is marked superseded -- the full history is preserved with its ABHA ID linkage intact. In V2, Aayu will integrate directly with the ABDM HIE-CM gateway, so consent artifacts are issued and managed by India's national health consent infrastructure -- the same system trusted by 45 crore ABHA holders.
Workflow
Forward a prescription photo, lab PDF, voice note, or text message via Telegram bot
Gemini extracts structured data. Each record is tagged with ABHA ID and ABDM record type automatically
Claude checks drug interactions, detects gaps, and generates the 7 AM morning briefing
Alerts arrive on Telegram. The full health picture is available on the web dashboard at any time
System Design
Inputs (raw data sources)
One ABHA ID -- all hospitals, all doctors, all records linked
Gemini Flash parses -- Claude Sonnet reasons across all data
Immutable log: who accessed what, when, and what changed
Outputs (what the caregiver sees)
Your personal health data is never shared without your explicit permission.
Security and Privacy
Every record transfer requires your explicit consent via ABDM's HIE-CM Consent Manager. No data moves without you approving it -- not to doctors, not to insurance companies, not to us.
Aayu uses India's National Health Authority (NHA) gateway. Your records link to ABHA -- the same system used by AIIMS and 438,000+ hospitals across the country.
Health data is Sensitive Personal Data under India's Digital Personal Data Protection Act 2023. We process it with explicit purpose consent, retain only what's needed, and provide full deletion on request.
When a prescription changes, the old version is marked superseded -- not erased. Every version is preserved with a timestamp and its full ABHA ID linkage intact.
Data is encrypted at rest and in transit (AES-256, TLS 1.3). Only family members you explicitly add can view records. Every access is logged with a timestamp and actor ID.
Real stories
"The drug interaction alert caught something my father's own doctors had missed. His cardiologist and neurologist didn't know what the other had prescribed. Aayu flagged it the morning after I forwarded the second prescription."

"I used to call my mother every morning to verify her medicines. Now the 7 AM briefing does it. I call her to talk, not to audit her prescriptions."
